The requirements are as follows:

A foreigner must submit:

• a copy of a passport with a photograph
• a copy of a valid visa, or residence permit or another document permitting their stay at the area of the Republic of Poland
• a copy of an insurance policy (if the applicant has one)
• a Certificate in Secondary Education (an original and its Polish certified translation)

• which includes a clause stating the right to apply for studies to all types of schools providing higher education in the country issuing the certificate or, in case of a lack of such a clause, a certificate confirming such a right issued by:
• educational authorities of the country where the certificate was issued, or
• a diplomatic post accredited at the area of the Republic of Poland or a consular post of the country where the certificate was issued, or
• the consul of the Republic of Poland in the country which issued the certificate, that is with a clause of Polish diplomatic post stating the right to admit to the studies in the country of residence analogically to the studies you wish to complete in Poland, or
• a certificate about nostrification of a Certificate in Secondary Education issued by the Board of Education which is right for the area of the school admitting the foreigner

• a medical certificate confirming good state of health or a lack of contraindications for studying
• an application form and 4 /four/ signed photographs
• a statement concerning citizenship and a school fee

All documents must be in Polish; in case of documents in a foreign language, a certified translation is required.
The application form must be filled in block letters.
